FULL THROTTLE AUTO

PAYMENT GUARANTEE AND MECHANIC'S LIEN NOTICE:

I, the undersigned, hereby authorize Full Throttle Auto to perform the services described above on the vehicle identified in this document. I understand and agree to the following terms:

1. AUTHORIZATION: I authorize Full Throttle Auto to perform diagnostic, repair, and service work as described above and to obtain parts necessary to complete said work.

2. ESTIMATE AND APPROVAL: I understand that the estimate provided is an approximation of costs. If additional work is discovered that will exceed the estimate by more than $50 or 10% (whichever is greater), Full Throttle Auto will contact me for approval before proceeding with such work.

3. PAYMENT TERMS: I agree to pay Full Throttle Auto in full for all services rendered and parts supplied upon completion of work. Payment is due at the time the vehicle is picked up. Accepted payment methods include cash, check, and major credit cards.

4. MECHANIC'S LIEN: I understand that Full Throttle Auto retains a mechanic's lien on the vehicle for the amount of all services performed and parts supplied. If payment is not received within 30 days of service completion, Full Throttle Auto has the legal right to retain possession of the vehicle and may pursue collection through legal means, including potential sale of the vehicle to satisfy the debt, in accordance with state law.

5. LIABILITY: I understand that Full Throttle Auto will exercise reasonable care in performing services. However, I release Full Throttle Auto from liability for loss or damage to the vehicle or articles left in the vehicle in case of fire, theft, or any other cause beyond Full Throttle Auto's control.

6. PERSONAL PROPERTY: Full Throttle Auto is not responsible for loss of or damage to personal property left in the vehicle.
